Default Play Call Sheet Template

I am looking for a template that you guys may have and use for games. We numbered our plays last year and now I am looking for a more efficient way to do it. We might not run in plays every down and we are looking for a way to call in plays using a wrist band on the QB and off my play list. Any ideas?
